Context

What situation forced a decision? Describe the problem, the constraints (technical, clinical, regulatory, cost, team size, deadlines) and the forces pulling in different directions. Link the Plan, spike, RCA or PR that surfaced it. Write it so that someone joining the team in a year can follow it without asking.

What we decided, stated plainly in the active voice: "We will ...". Include the scope: which apps, services or data it applies to, and anything it explicitly does not cover.
